ABSTRACT

La gota es el trastorno del metabolismo de las purinas que se caracteriza por acumulación de ácido úrico por aumento de su producción o por disminución de su excreción. Con el paso del tiempo, el exceso de urato monosódico permite que se deposite en diferentes tejidos del organismo; siendo particularmente infrecuente la presentación de tofos a nivel facial. Paciente masculino adulto de 56 años, con antecedente de gota hace 38 años y presencia de tofos gotosos a nivel de miembros superiores e inferiores que acude por cuadro de 4 años de evolución de lesión indurada, levemente dolorosa a nivel de tercio medio de dorso nasal que ha ido aumentando de tamaño, que causa deformidad de pirámide nasal y leve sensación de obstrucción nasal bilateral por lo que se planifica resolución quirúrgica. El análisis histopatológico de las muestras enviadas bajo exéresis quirúrgica confirma que tejido enviado corresponde a tofo gotoso. Los tofos gotosos pueden aparecer en diferentes tejidos, como cartílagos, membranas y líquido sinovial, superficies articulares, siendo excepcionalmente raro, en el miocardio, válvulas mitral y aórtica, ojos, nariz y médula espinal. El lugar de presentación puede ser muy variable, al igual que su tamaño


Gout is a purine metabolism disorder characterized by accumulation of uric acid due to increased production or decreased excretion. Over time, excess monosodium urate allows it to be accumulated in different body tissues, although the occurrence in the facial area is particularly infrequent. A 56- year-old male patient with a gout antecedent from 38 years ago that presented gouty tophi at the level of the upper and lower limbs seek medical advice due to an indurated slightly painful lesion at the level of the middle third of the nasal dorsum that started 4 years before and has been increasing in size. The lesion was causing nasal pyramid deformity and a slight sensation of bilateral nasal obstruction, for which surgical resolution is planned. The histopathological analysis of the samples sent under surgical exeresis confirms that the tissue sample corresponds to gouty tophi. Gouty tophi can appear in different tissues, such as cartilage, membranes and synovial fluid, joint surfaces, being exceptionally rare in the myocardium, mitral and aortic valves, eyes, nose and spinal cord. The place of presentation can be very variable, as well as its size

SUMMARY: The latissimus dorsi is a broad muscle that originates from the inferior thoracic spinous processes, thoracolumbar fascia, iliac crest, and inferior ribs. It inserts on the inferior aspect of the intertubercular groove of the humerus through a thin tendon. The study was conducted on 10 cadavers (7 male and 3 female). These specimens were dissected and examined to study the gross anatomical characteristics of the latissimus dorsi muscle. The dimensions of the latissimus dorsi muscle and its surface area were measured in all the cadavers. The branching pattern of the thoracodorsal vessels was recorded. The pedicle length and caliper were measured using Vernier calipers. On the 20 dissected sides, the thoracodorsal artery was found to be one of the terminal branches of the subscapular artery that originates in the axillary region. In 19 (95 %) cases, the thoracodorsal artery terminated in a bifurcation, giving off a medial and a lateral branch. The average size of the elevated flap of the latissimus dorsi muscle was 18 cm x 36 cm. The average pedicle length was 9.5 cm (range: 5 cm-14 cm), and the average diameter at its origin was 2.5 mm (range: 1.5 mm-3.5 mm). The average diameter of the vena comitans was 3.3 mm. The current study focuses on the anatomical features of the latissimus dorsi muscle and its blood supply to increase the success rate of operations in clinical practice.


RESUMEN: El músculo latísimo del dorso se origina en los procesos espinosos de las vértebras torácicas inferiores, la fascia toracolumbar, la cresta ilíaca y las costillas inferiores y se inserta en el surco intertubercular del húmero a través de un delgado tendón. El estudio se realizó en 10 cadáveres (7 mujeres y 3 hombres). Estos especímenes fueron disecados y examinados para estudiar las características anatómicas macroscópicas del músculo latísimo del dorso. En todos los cadáveres se midieron las dimensiones del músculo y su superficie. Se registró el patrón de ramificación de los vasos toracodorsales. La longitud del pedículo y el calibre se midieron con paquímetro Vernier. En los veinte lados disecados, se encontró que la arteria toracodorsal era una de las ramas terminales de la arteria subescapular que se originaba en la región axilar. En 19 (95 %) casos, la arteria toracodorsal terminaba bifurcándose en dos ramas, una rama medial y otra lateral. El tamaño promedio del colgajo elevado del músculo latísimo del dorso era de 18 cm x 36 cm. La longitud promedio del pedículo era de 9,5 cm (rango: 5 cm-14 cm), y el diámetro promedio en su origen era de 2,5 mm (rango: 1,5 mm-3,5 mm). El diámetro medio de la vena comitans era de 3,3 mm. El estudio actual se centra en las características anatómicas del músculo latísimo del dorso y su irrigación para aumentar la tasa de éxito de las operaciones en la práctica clínica.

Abstract Introduction Augmentation rhinoplasty depends mainly on intact stable bony and cartilaginous parts. Many trials have used different materials as a graft to perform the operation and support the nose. Debate exists whether alloplastic or autogenic grafts are more appropriate. Common available alloplastic grafts include silicone, medpor, and gore-tex. Autogenic grafts are usually derived from costal cartilages. Warping, infection, and hypertrophic scars are the main complications of the procedure. Yet no subgroup analysis has been performed to investigate the effect of different risk factors. Objective To investigate the effect of different types of grafts and the association of the income level of the country on surgery complications. Methods A comprehensive literature search of articles was conducted in PubMed, Cochrane Library, Web of Science, and SCOPUS databases through October 2019. We included articles that used autologous or alloplastic grafts in nasal dorsum reconstruction surgery. We performed subgroup analysis according to the type of graft used, region, and income level of the country. A meta-regression analysis model was carried out from the period of 1999-2018, to study the incidence of these complications over time. Results The overall complication rate was 7.1%, which was higher in the alloplastic group (7.8%) than the autogenic group (6.9%). The most common complications were secondary surgery for re-correction (4.1%), infection (2.1%), warping (1.6%), and hypertrophic scars (1.6%). All outcomes were homogeneous (I2 < 50%). Conclusion Patients with autogenic grafts are less liable to develop complications than their peers reconstructed with alloplastic grafts. Moreover, Asian patients are less susceptible to overall rhinoplasty complications. Attention should be noted for low-income countries in which surgical complications are more prone to occur.


Resumo Introdução A feitura de uma rinoplastia de aumento depende principalmente das partes ósseas e cartilaginosas intactas. Muitos estudos usaram enxertos de diferentes materiais para a feitura da cirurgia e como apoio da estrutura nasal. Ainda existem controvérsias em estudos prévios sobre quais tipos de enxertos, se materiais aloplásticos ou autogênicos, seriam os mais adequados. Os enxertos aloplásticos comuns incluem silicone, medpor e gore-tex. Os enxertos autogênicos são geralmente derivados de cartilagens costais. Deformações, infecção e cicatrizes hipertróficas são as principais complicações do procedimento. No entanto, nenhuma análise de subgrupo foi feita para investigar o efeito de diferentes fatores de risco. Objetivo Investigar o efeito de diferentes tipos de enxertos e o nível de renda do país nas complicações cirúrgicas Método Uma pesquisa abrangente de artigos na literatura foi feita nas bases de dados PubMed, Cochrane Library, Web of Science e SCOPUS até outubro de 2019. Foram incluídos artigos que usaram enxertos autólogos ou aloplásticos em cirurgias de reconstrução do dorso nasal. Foi feita uma análise de subgrupos de acordo com o tipo de enxerto usado, região e nível de renda do país. Um modelo de análise de metarregressão foi feito de 1999 a 2018, para estudar a incidência dessas complicações ao longo do tempo. Resultados A taxa global de complicações foi de 7,1%, a qual foi maior no grupo aloplástico (7,8%) do que no grupo autogênico (6,9%). As complicações mais comuns foram cirurgia secundária para recorreção (4,1%), infecção (2,1%), deformidade (1,6%) e cicatrizes hipertróficas (1,6%). Todos os resultados foram homogêneos (I2 < 50%). Conclusão Os pacientes com enxertos autogênicos são menos propensos a desenvolver complicações, em comparação com seus pares com enxertos aloplásticos. Além disso, pacientes asiáticos são menos suscetíveis a complicações gerais da rinoplastia. Merece atenção o fato de que em países de baixa renda as complicações cirúrgicas são mais propensas a ocorrer.

SUMMARY: The anatomical variations of the pectoralis major muscle (PMM) and latissimus dorsi muscle (LDM) range from agenesis to the presence of supernumerary fascicles with a variety of insertions and relationships with the muscles, fascia, vessels, nerves and skeletal components of the shoulder girdle and the axilla. Many of these variations are clinically irrelevant, but extremely interesting and revealing from the perspective of comparative anatomy, ontogeny, and phylogeny. In this report, we present two different supernumerary muscles in the chest of one adult male body, identified during dissection practice of undergraduate medical students at Universidad Surcolombiana. These supernumerary fascicles in the axillary region were caudal to the lower edge of the PMM on the right side of the chest, and in contact with the anterior edge of the LDM on the left side of the chest; each fascicle was inserted in the ipsilateral coracoid process. These observations are congruent with the pectoralis quartus muscle and an incomplete and superficial axillary arch, respectively.


RESUMEN: Las variaciones anatómicas del músculo pectoral mayor (MPM) y del músculo latísimo del dorso (MLD) van desde la agenesia hasta la presencia de fascículos supernumerarios con una variedad de inserciones y relaciones con los músculos, fascias, vasos, nervios y componentes esqueléticos de la cintura escapular y la axila. Clínicamente, muchas de las variaciones son irrelevantes, pero extremadamente interesantes y reveladoras desde la perspectiva de la anatomía comparada, la ontogenia y la filogenia. En este estudio, presentamos dos músculos supernumerarios diferentes en la pared torácica de un hombre adulto, identificados durante la práctica de disección de estudiantes de medicina de la Universidad Surcolombiana. Estos fascículos supernumerarios en la región axilar estaban caudales al margen inferior del MPM en el lado derecho del tórax y en contacto con el margen anterior del MLD en el lado izquierdo del tórax; cada fascículo se insertaba en el proceso coracoides ipsilateral. Estas observaciones son congruentes con el músculo pectoral cuarto y un arco axilar incompleto y superficial, respectivamente.

En este manuscrito se revisan las diferentes causas que producen aumento de la cifosis torácica (dorso curvo), específicamente en niños y adolescentes. Las causas del dorso curvo que se analizan en este artículo son: Dorso curvo postural, idiopático, neuromuscular, congénito y enfermedad de Scheuermann. Se centra en los factores que producen su aparición, características de su evolución y tratamiento.


This manuscript reviews the different causes that lead to increased thoracic kyphosis, specifically in children and adolescents. The causes of increased thoracic kyphosis that will be discussed in this article are: postural, idiopathic, neuromuscular, congenital, and Scheuermann's disease. This paper focuses on the factors that produce its appearance, characteristics of its evolution, and treatment.

Resumen El 5 de octubre del 2019 se capturó un individuo de Catharus fuscescens, como parte de los muestreos mensuales del proyecto Monitoreo de las poblaciones de avifauna en el departamento de Loreto - Perú realizado por el Observatorio de Aves Loreto - LBO en la comunidad de San Rafael, distrito de Indiana, departamento de Loreto. Esta captura representa el primer registro documentado de C. fuscescens para el departamento de Loreto, confirmando la condición sugerida de migrante raro en el extremo noreste del Perú en su paso hacia el sur durante su migración hacia Sudamérica.


Abstract On October 5, 2019, an individual from Catharus fuscescens was captured, as part of the monthly sampling of the project Monitoring of birdlife populations in the department of Loreto - Peru conducted by the Loreto Bird Observatory - LBO in the San Rafael community, Indiana district, department of Loreto. This capture represents the first documentation of C. fuscescens for the department of Loreto, confirming the suggested condition of a rare migrant in the northeastern end of Peru as it moves south as its migration season to South America.

RESUMEN: El músculo extensor corto de los dedos (ECD) se sitúa junto con el músculo extensor corto del hallux (ECH) en la región dorsal del pie y están encargados de colaborar con la acción agonista de los músculos extensor largo de los dedos (ELD) y extensor largo del hallux (ELH), en la extensión de los cuatro dedos mediales en las articulaciones metatarsofalángicas. Esta condición complementaria permite su transferencia quirúrgica hacia otras regiones receptoras sin afectar la funcionalidad extensora de los dedos del pie motivo por el cual durante las últimas décadas se ha convertido en un tejido importante para la reconstrucción de lesiones tisulares, sin embargo, son escasos los estudios biométricos que lo describan en detalle. El objetivo de esta investigación fue establecer la longitud, ancho y área de extensión de estos músculos acompañada de la determinación de las distancias de éstos respecto a la cuña medial y la base del quinto metatarsiano para su uso quirúrgico. Sumado a lo anterior y posterior a la descripción de las variantes musculares encontradas se determinó los puntos motores de inervación de estos dos músculos. Para ello se estudiaron 36 miembros inferiores formolizados de individuos adultos brasileños pertenecientes a la Universidad Federal de Alagoas (UFAL), Maceió, Brasil. Las mediciones se realizaron con un cáliper marca Mitutoyo de 0,01 mm de precisión obteniendo una longitud, ancho y área de 6,89 ± 1,64 cm, 3,81 ± 0,42 cm y 31,98 ± 7,60 cm2 en el lado derecho de 6,91 ± 1,64 cm, 3,68 ± 0,46 cm y 30,75 ± 7,61 cm2 en el izquierdo respectivamente. En el 17 % de los casos hay presencia de tendones accesorios para el músculo ECD. La distancia desde el margen medial del músculo ECH respecto a la cuña medial y del margen lateral del músculo ECD a la base del quinto metatarsiano fue de 1,97 ± 0,43 y 1,72 ± 0,41 al lado derecho y de 2,01 ± 0,62 y 1,87 ± 0,36 al lado izquierdo respectivamente. Los puntos motores (Pm) predominaron en un 64 % en el tercio medio del músculo ECH y en un 64 % en el tercio proximal del músculo ECD. Estos resultados son un aporte significativo, tanto para quienes realizan cirugía ortopédica como para el conocimiento detallado de la anatomía dorsal del pie.


SUMMARY: The extensor digitorum brevis muscle (EDB) is located along with the extensor hallucis brevis (EHD) in the dorsal region of the foot and are responsible for collaborating with the agonist action of the extensor digitorum longus muscles (EDL) and extensor hallucis longus (EHL) in the extension of the four medial fingers in the metatarsophalangeal joints. This complementary condition allows its surgical transfer to other receptor regions without affecting the extensor functionality of the toes, which is why during the last decades it has become an important tissue for the reconstruction of tissue injuries, however, there are few studies biometrics that describe it in detail. The objective of this investigation was to establish the length, width and area of extension of these muscles accompanied by the determination of their distances from the medial wedge and the base of the fifth metatarsal for surgical use. In addition to the above and after the description of the muscle variants found, the innervation motor points of these two muscles were determined. To do this, 36 formalized lower limbs of Brazilian adult individuals belonging to the Federal University of Alagoas (UFAL), Maceió, Brazil, were studied. Measurements were made with a 0.01 mm precision Mitutoyo caliper obtaining a length, width and area of 6.89 ± 1.64 cm, 3.81 ± 0.42 cm and 31.98 ± 7.60 cm2 on the right side of 6.91 ± 1.64 cm, 3.68 ± 0.46 cm and 30.75 ± 7.61 cm2 on the left, respectively. In 17 % of cases there is presence of accessory tendons for the EDB muscle. The distance from the medial margin of the EHB muscle with respect to the medial wedge and the lateral margin of the EDB muscle to the base of the fifth metatarsal was 1.97 ± 0.43 and 1.72 ± 0.41 on the right side and 2,01 ± 0.62 and 1.87 ± 0.36 on the left side respectively. Motor points (Pm) predominated in 64 % in the middle third of the EHB muscle and in 64 % in the proximal third of the EDB muscle. These results are a significant contribution both for those who perform orthopedic surgery and for detailed knowledge of the dorsal foot anatomy.

ABSTRACT

In this review we supply information on medical methods for thoraco-lumbar spine fracture management, their efficiency and complication rates, based on previously published researches and also give background information on epidemiology and classification of thoraco-lumbar fractures. We conducted a narrative review over the literature using electronic databases as; MEDLINE, and EMBASE for studies involving data on Dorso-lumbar Spine traumatic injuries, published in September 2019. Spine fractures account for a large portion of musculoskeletal injuries worldwide. A classification of back cracks is essential in order to establish a typical language for therapy indicators and results. Clinical exam, mechanism of injury, and imaging are heavily trusted to choose regarding medical versus non-surgical management.

Resumen Introducción: La estimulación magnética transcraneal (EMT) es una opción potencial de tratamiento para la sintomatología de trastorno del espectro autista. Objetivos: Determinar la eficacia de la EMT en modalidad theta-burst intermitente (ETBi) sobre la corteza pre frontal dorso lateral izquierda (CDLPF) en el manejo de comportamientos patológicos asociados con TEA. Pacientes y Métodos: Paciente masculino de 10 años de edad diagnosticado con TEA con (ADOS) Autistic Diagnostic Observation Schedule, presenta una historia familiar de una hermana menor con TEA. Tanto el estudio de Resonancia Magnética encéfalo (IRM) y IRM funcional (IRMf) con paradigma emocionales se efectuaron antes del tratamiento de ETBi, inmediatamente después de tratamiento ETBi, y después de 6 meses de tratamiento con ETBi. Resultados: La prueba de ATEC reporto mejoría en las escalas de comunicación, cognición, social y comportamiento, 83%,81%,72% y 52% respectivamente. La puntuación total mejoro un 66% inmediatamente después de tratamiento y 55% en un periodo de 6 meses. La IRMf revelo la activación de la corteza frontal, parietal y occipital antes del tratamiento con ETBi y persistiendo la mejoría por 6 meses. Conclusión: ETBi una técnica de neuromodulación no invasiva bien tolerado que requiere un tiempo de administración menor que el tratamiento estándar.


Abstract Background: Transcranial magnetic stimulation (TMS) is a potential treatment option for autism spectrum disorder (ASD) symptomatology. Objectives: To determine the efficacy of the TMS intermittent theta-burst stimulation (iTBS) protocol over the left dorsolateral prefrontal cortex (DLPFC) in the management of pathological behaviors associated with ASD. Patient/Methods: A 10-year-old male diagnosed with ASD by the Autism Diagnostic Observation Schedule (ADOS) presented with a family history of ASD with a younger sister diagnosed with the same disorder. Both magnetic resonance imaging (MRI) and functional MRI (fMRI) scans for emotional paradigms were performed before the iTBS treatment, immediately after the iTBS treatment, and after 6 months of iTBS treatment. Results: The ATEC reports revealed improvement in communication, cognition, sociability, and behavior scales by 83%, 81%, 72%, and 52%, respectively. The overall score improved by 66% immediately after the treatment and by 55% lasting over a 6-month period. The fMRI revealed the activation of the frontal, parietal, and occipital cortex before iTBS treatment, and a better integration and activation of the frontal, temporal, and occipital cortex after iTBS treatment and persisted after 6 months. Conclusions: iTBS is a well-tolerated, non-invasive neuromodulation technique that requires relatively less administration than the standard treatment.

Abstract Introduction: In rhinoplasty, the nasal dorsum has important relevance regarding the esthetic and functional aspects of the surgery. Its reduction should be performed with maximum accuracy and controlled resection to prevent or minimize potential complications. The septum pyramidal adjustment and repositioning consists of a conservative surgical technique of the nasal dorsum, which does not require the detachment of the upper lateral cartilages of the nasal septum, allowing the remodeling of the nasal dorsum while maintaining esthetic lines and nasal function, potentially reducing frequent complications in more traditional surgeries. Objective: To describe the septum pyramidal adjustment technique in detail, presenting its advantages and disadvantages in relation to the other surgical approaches, as well as to disclose results of this surgical procedure in patients submitted to primary rhinoplasty in a specific hospital. Methods: The medical records of all patients submitted to surgery from 2011 to 2015 through this surgical technique were evaluated by the same team. Of these cases, certain variables were analyzed such as gender, age, indication for reoperation and surgical complications. Results: 153 patients underwent rhinoplasty through septum pyramidal adjustment. Of these, 13 patients experienced an indication for a second surgery and four had some type of postoperative complication. Conclusion: The septum pyramidal adjustment surgical technique is a simple procedure, as it does not require the reconstruction of the nasal dorsum. It has a low number of complications and preserves the anatomical structures.


Resumo Introdução: Na rinoplastia, o dorso nasal tem importante relevância no quadro estético e funcional. A sua redução deve ser realizada com máxima precisão e ressecção controlada a fim de prevenir ou minimizar complicações potenciais. O termo septum pyramidal adjustment and repositioning consiste em uma técnica cirúrgica conservadora do dorso nasal, que não requer a desinserção das cartilagens laterais superiores do septo nasal, que permite remodelar o dorso nasal, manter as linhas estéticas e a função nasal, reduz potencialmente complicações frequentes nas cirurgias mais tradicionais. Objetivo: Descrever em detalhes o septum pyramidal adjustment, expor suas vantagens e desvantagens em relação às outras abordagens cirúrgicas, bem como apresentar resultados desse procedimento cirúrgico em pacientes submetidos à rinoplastia primária em um hospital específico. Método: Foram avaliados os registros médicos de todos os pacientes operados de 2011 a 2015 por essa técnica cirúrgica, pela mesma equipe. Desses casos, foram analisadas algumas variáveis, como: sexo, idade, indicação de reoperação e complicações cirúrgicas. Resultados: Foram submetidos 153 pacientes à rinoplastia por septum pyramidal adjustment. Desses, 13 tiveram segunda indicação cirúrgica e quatro apresentaram alguma complicação pós-operatória. Conclusão: A técnica cirúrgica septum pyramidal adjustment apresenta-se como um procedimento de fácil realização, pois não exige a reconstrução do dorso nasal. Apresenta baixo número de complicações e preserva as estruturas anatômicas.

Abstract Introduction: Following nasal hump removal during septorhinoplasty, the middle vault should be reconstructed to avoid functional and esthetic problems. Middle vault reconstruction, however, may result in widening of the middle vault and may need a camouflage graft to cover dorsal irregularities. Objective: To present the results of reconstructing the middle vault with a technique that covers the nasal dorsum with upper lateral cartilage, from the viewpoint of patient satisfaction. Methods: Retrospective study of patients who underwent septorhinoplasty that included nasal dorsum closure with upper lateral cartilage from December 1, 2014 to January 31, 2016. Those with postoperative follow-up of less than 3 months were excluded. The final study group included 39 patients. The same surgeon performed all septorhinoplasties. The dorsum was closed using an "upper lateral closing" technique that approximated upper lateral cartilages to each other over the septum. Postoperative patient satisfaction was determined using a visual analog scale and the rhinoplasty outcomes evaluation questionnaire. The questionnaire evaluates patient esthetic and functional satisfaction with the operated nose. High scores indicate improved esthetic results. Results: No dorsal irregularities were seen at postoperative follow-up evaluation of the patients. For esthetic nasal appearance, the median visual analogue scale scores was 86%, and the mean for the questionnaire was 77.03%. Conclusion: The natural dome-shaped anatomy of the nasal dorsum was achieved by approximating the upper lateral cartilages to each other. Closing the dorsum with this technique also covers any dorsal irregularities and results in a smooth dorsum. Patients expressed satisfaction with the esthetic and functional aspects of the smooth, attractive nasal dorsum.


Resumo Introdução: Após a remoção da giba nasal durante a rinosseptoplastia, a abóbada deve ser reconstruída para evitar problemas funcionais e estéticos. A reconstrução da abóboda entretanto, pode resultar em alargamento dorsal e pode necessitar de um enxerto para camuflar irregularidades dorsais. Objetivo: Avaliar a satisfação dos pacientes com os resultados da reconstrução da abóboda com uma técnica que utiliza a cartilagem lateral superior para recobrir o dorso nasal. Método: Estudo retrospectivo de pacientes submetidos a rinosseptoplastia, que incluiu fechamento do dorso nasal com cartilagem lateral superior, realizado de 1º de dezembro de 2014 a 31 de janeiro de 2016. Foram excluídos aqueles com acompanhamento pós-operatório de menos de 3 meses. O grupo final do estudo incluiu 39 pacientes. O mesmo cirurgião realizou todas as rinosseptoplastias. O dorso foi fechado com uma técnica de "fechamento lateral superior" que aproxima as cartilagens laterais superiores de cada lado sobre o septo. A satisfação pós-operatória dos pacientes foi determinada através de uma escala visual analógica e o questionário Rhinoplasty Outcomes Evaluation. O questionário avalia a satisfação estética e funcional do paciente com o nariz. Escores altos indicam percepção de melhoria estética. Resultados: Não foram observadas irregularidades dorsais na avaliação pós-operatória de seguimento dos pacientes. Em relação à aparência nasal estética, o escore médio da escala visual analógica foi 86% e o escore médio do questionário foi 77,03%. Conclusões: A anatomia natural em forma de domo do dorso nasal foi conseguida através da aproximação das cartilagens laterais superiores entre si. O fechamento do dorso com essa técnica também abrange todas as irregularidades dorsais e resulta em um dorso liso. Os pacientes expressaram satisfação com os aspectos estéticos e funcionais do dorso nasal liso e atraente.

RESUMEN Introducción: Entre las alteraciones del dorso nasal puede encontrarse desde un rádix bajo hasta un aplastamiento severo del dorso nasal, siendo fundamental el uso de injertos y material de relleno. Objetivo: Proponer una nueva alternativa de relleno y camuflaje en rinoplastía mediante un injerto mixto de L-PRF más cartílago picado en una matriz de fibrina autóloga. Material y método: Evaluación de los pacientes operados de rinoseptoplastía en quienes se utilizó el injerto mixto entre febrero y septiembre del año 2016 en el Servicio de Otorrinolaringología del Hospital Regional de Concepción. Evaluación de resultados respecto a reabsorción e irregularidades del injerto y autoevaluación del grado de satisfacción. Resultados: Fueron operados 7 pacientes. Cuatro de ellos presentaban aplastamiento severo de dorso nasal y tres un rádix profundo. Se logró el relleno y camuflaje del área deseada en todos los casos. No se presentó desplazamiento ni reabsorción del injerto en ninguno de los casos. El uso de este injerto no significó mayor costo económico, y el grado de satisfacción fue alto en todos los casos. Conclusión: Primer estudio en utilizar un injerto mixto de cartílago más L-PRF en una matriz de fibrina autóloga en rinoplastía, demostrando ser una alternativa asequible, efectiva, reproducible y segura, particularmente útil en casos de revisión.


ABSTRACT Introduction: Among the alterations of the nasal dorsum there can be found from a low radix to a severe crushing of the nasal dorsum, being fundamental the use of grafts and filling materials. Aim: To propose a new filling and camouflage alternative in rhinoplasty by means of a mixed graft of L-PRF and chopped cartilage in an autologous fibrin matrix. Material and method: Evaluation of the operated patients of rhinoseptoplasty in whom the mixed graft was used between February and September 2016 in the Otorhinolaryngology unit of the Hospital Regional de Concepción. Evaluation of results regarding resorption and graft irregularities and self-assessment of degree of satisfaction. Results: Seven patients were operated. Four of them presented severe crushing of the nasal dorsum and three of them a deep radix. The filling and camouflage of desired area was achieved in all cases. There were no displacement or resorption of the graft in any case. The use of this graft did not mean greater economic cost, and the degree of satisfaction was high in all cases. Conclusions: First study to use a mixed graft of cartilage and L-PRF in an autologous fibrin matrix in rhinoplasty, proving to be an affordable, effective, reproducible and safe alternative, particularly useful in revision surgeries.

RESUMEN Introducción: La rinoseptoplastía es una cirugía compleja y desafiante. Permite resolver problemas estéticos y funcionales de la nariz. Objetivo: Describir la experiencia en rinoseptoplastías realizadas en el Comité de Nariz en nuestro centro. Material y método: Estudio descriptivo de pacientes sometidos a rinoseptoplastía entre 2013 y 2015. Resultados: Se operaron 265 pacientes en 3 años, 164 (61,8%) fueron mujeres y la edad promedio fue de 29 años (rango 13-73). Las cirugías fueron principalmente primarias (secundarias: 2,6%). El abordaje más utilizado fue abierto (92,5%). Se realizó trabajo de punta nasal en 253 (95,5%) pacientes; resección del dorso osteocartilaginoso en 252 (95,1%), y aumento de éste en sólo 5 (1,9%). Las osteotomías fueron frecuentes: paramedianas en 229 (86,4%), percutánea lateral bilateral en 217 (81,9%) y percutánea lateral unilateral en 17 (6,4%). El vástago columelar fue el injerto de punta más frecuentemente utilizado, en 241 (90,9%) pacientes, seguido del escudo en 69 (26,0%). En el dorso, se utilizaron autoespaciadores en 124 (46,7%) pacientes, y espaciadores en 109 (41,1%). Conclusiones: El abordaje abierto fue el más frecuente, demostrando ser favorable en nuestro centro por la exposición y aprendizaje de nuestros residentes. Se constató 2,6% de cirugías secundarias. Se destaca el uso de vástago columelar para otorgar soporte a la punta nasal dada la alta prevalencia de nariz hispana en nuestro centro.


ABSTRACT Introduction: Rhinoseptoplasty is a complex and challenging surgery. It addresses nasal aesthetics and functionality. Aim: To describe the experience in rhinoseptoplasty performed by the Nose Committee at our medical center. Material and method: Descriptive study, of the all rhinoseptoplasties performed by the Nose Committee between 2013 and 2015. Results: 265 patients underwent surgery in the 3-year period; 164 (61.8%) were women and the mean age was 29 years (range 13-73). Most were primary surgeries (secondary: 2.6%). An external approach was most commonly used (92.5%). Nasal tip surgery was performed in 253 (95.5%) patients; osteocartilaginous dorsum reduction in 252 (95,1%), and augmentation in only 5 (1,9%). Osteotomies were frequent: paramedian in 229 (86.4%), bilateral percutaneous lateral osteotomies in 217 (81.9%) and unilateral percutaneous lateral in 17 (6.4%). Regarding nasal tip grafts, columellar strut was most often used, in 241 (90,9%) patients, followed by the shield graft in 69 (26,0%). For the dorsum, autospreaders were used in 124 (46.7%), and spreader grafts in 109 (41.1%). Conclusions: Open rhinoseptoplasty was the most commonly used approach. This has shown favorable for residents due to better exposition of nasal anatomy. Only 2.6% were secondary rhinoseptoplasties. We emphasize the use of the columellar strut for nasal tip support as we frequently encounter hispanic noses at our center.

ABSTRACT

INTRODUCCIÓN: El fragmento dorso-ulnar (FDU) en la fractura intraarticular de radio distal es de especial importancia puesto que implica una alteración en la articulación radio-carpiana así como también en la articulación radio-ulnar distal (RUD), donde la incongruencia articular permanente puede generar secuelas a largo plazo. OBJETIVO: Proponer una clasificación del FDU, definiendo cuándo realizar el procedimiento quirúrgico con asistencia artroscópica, basado en una serie consecutiva de casos operados de fractura del radio distal estudiados con tomografía computada (TC). MÉTODO: Estudio descriptivo de una serie de casos de pacientes operados por fractura del radio distal entre enero del 2015 y diciembre del 2016. En base a eso, se elabora una clasificación del FDU y se sugiere un esquema de manejo específico. Se describe el FDU como aquel fragmento específico ubicado en la esquina dorso-ulnar de la carilla articular del radio distal, con compromiso de más del 30% de la superficie articular RUD y más de 5mm desde el borde ulnar hacia radial de la cortical dorsal del radio observado en el corte axial de la TC preoperatoria. Se considera un fragmento mayor (FM) cuando el rasgo de fractura compromete hacia radial hasta el tubérculo de Lister y se considera fragmento menor (Fm) cuando el rasgo no alcanza a comprometer el tubérculo de Lister. Nuestra propuesta de clasificación reconoce 4 tipos: tipo I (FM sin desplazamiento, en fracturas tipo C de la AO); tipo II (FM con desplazamiento, en fracturas tipo C de la AO); tipo III (Fm independiente del desplazamiento, en fracturas tipo C de la AO) y tipo IV (FM/Fm con desplazamiento, en fracturas tipo B2 de la AO). Esquema de manejo: Tipo I síntesis con placa bloqueada por abordaje palmar, sin obligación de asistencia artroscópica. Tipo II síntesis con placa bloqueada por abordaje palmar, con asistencia artroscópica requerida. Tipo III síntesis percutánea dorsal contornillo canulado, bajo asistencia artroscópica. Tipo IV síntesis dorsal con placa o tornillo mediante abordaje dorsal bajo visión directa o con asistencia artroscópica, usando portales artroscópicos volares. RESULTADOS: Se operaron 488 fracturas de radio distal durante el período mencionado; 375 fracturas clasificadas como tipo C de la AO. Del total operadas, solo 392 fracturas contaban con TC peroperatoria, que permitía evaluar la presencia del FDU, el cual estuvo presente en 127/392 de los casos (32,4%). Analizados por grupo, 38 casos presentaban fragmentos tipo I, 22 tipo II, 69 tipo III y 7 tipo IV. DISCUSIÓN: El FDU se presentó en un 32,4% de los casos evaluables por TC en nuestra serie. El manejo dirigido de este fragmento con asistencia artroscópica permitió una reducción anatómica con fijación estable específica de éste. CONCLUSIÓN: Proponemos una nueva clasificación del FDU basada en la TC preoperatoria que permite realizar un adecuado plan prequirurgico y abordar este fragmento de manera específica sugiriendo cuando utilizar asistencia artroscópica.


INTRODUCTION: The dorsal-ulnar fragment (DUF) in the distal radius fracture is of special importance since it implies an alteration in the radio-carpal joint as well as in the distal radio-ulnar joint (DRUJ), where permanent joint incongruence can generate long-term sequelae. OBJECTIVE: To propose a classification of the DUF, advising when to perform arthroscopic assistance, based on a consecutive series of operated cases of distal radius fracture studied with computed tomography (CT). METHODS: Descriptive study of a series of cases of patients operated of distal radius fracture between January 2015 and December 2016. We describe a classification of the DUF and suggest a specific treatment scheme. The DUF is described as that specific fragment located in the dorso-ulnar corner of the articular surface of the distal radius, which involves more than 30% of the articular surface of the DRUJ and more than 5mm of the ulnar edge of the dorsal cortex of the radius observed in the axial section of the preoperative CT. It is considered a major fragment (FM) when the fracture compromises the Lister tubercle and is considered a minor fragment (Fm) when it does not. Our classification recognizes 4 types of DUF: type I (FM without displacement, in type C fractures of the AO); Type II (FM with displacement, in type C fractures of the AO); Type III (Fm independent of displacement, in fractures type C of the AO) and type IV (FM/Fm with displacement, in fractures type B2 of the AO). Treatment scheme: Type I: synthesis with a palmar locked plate without arthroscopic assistance required. Type II: synthesis with palmar locked plate with arthroscopic assistance. Type III dorsal percutaneous synthesis with cannulated screw with arthroscopic assistance. Type IV dorsal synthesis with plate or screw by dorsal approach under direct vision or with arthroscopic assistance using volar portals. RESULTS: A total of 488 distal radius fractures were operated during this period. Only 392 fractures had preoperative CT, which allowed to evaluate the presence of the DUF. It was present in 127/392 of the cases (32.4%). Analyzed by group, 38 cases presented fragments type I, 22 cases type II, 69 cases type III and 7 cases type IV. DISCUSSION: The DUF was presented in 32.4% of the cases in our series. The management of this fragment with arthroscopic assistance allowed an anatomical reduction with specific stable fixation of this fragment. CONCLUSION: We propose a novel classification of the DUF based on preoperative CT that allows a specific management of this fragment and suggest when to use arthroscopic assistance.

Introdução: Desde a década de 1980, com Illouz, a lipoaspiração ganhou popularidade e representa hoje um dos procedimentos mais realizados no mundo. Algumas de suas complicações são graves e potencialmente letais. Não existe, contudo, uma uniformidade em sua prática ou no seu ensino. A avaliação das técnicas empregadas por cirurgiões plásticos pode ser o início de uma padronização. Métodos: Foi aplicado um questionário sobre lipoaspiração no 52º Congresso Brasileiro de Cirurgia Plástica para cirurgiões plásticos de diferentes faixas etárias e regiões do Brasil, presentes no evento. Resultados: Foram contabilizados 243 questionários preenchidos (n = 243). O número médio de incisões foi de 9 (2 - 16). Duzentos e quarenta e um cirurgiões (99%) realizam incisões na linha mediana/ paramediana anteriormente e 236 (97%) incisam na linha mediana/paramediana na região posterior. Aproximadamente metade dos questionados utilizam a anestesia geral. Duzentos e nove cirurgiões (86%) posicionam o paciente em decúbito ventral durante o procedimento. A lipoaspiração superficial é realizada por 146 (60%) entrevistados, sendo que 22 (9%) fazem a aspiração apenas desta camada adiposa. Oitenta e cinco (35%) participantes relatam controlar a pressão do aparelho durante o procedimento. Conclusão: A lipoaspiração realizada no Brasil apresenta grande variação técnica. Essa constatação nos faz refletir sobre a necessidade de uma uniformização de sua prática e ensino a fim de aumentar o controle e a segurança do procedimento.


Introduction: Since the 1980s, with Illouz, liposuction has gained popularity and represents one of the most commonly performed procedures in the world today. Some of the complications are serious and potentially lethal. Nevertheless, uniformity in its practice or the manner in which it is taught does not exist. Evaluating techniques employed by plastic surgeons may be the start toward standardization. Methods: A questionnaire on liposuction was given to plastic surgeons of different age groups and from regions of Brazil who were present at the 52nd Brazilian Conference for Plastic Surgery. Results: Two hundred forty-three questionnaires were filled out (n = 243). The average number of incisions was 9 (2­16). Two hundred fortyone surgeons (99%) made incisions along the anterior median/ paramedian line, and 236 (97%) made incisions on the posterior median/paramedian line. Approximately half of those surveyed utilized general anesthesia. Two hundred nine surgeons (86%) placed the patient in the prone position during the procedure. One hundred forty-six (60%) interviewees performed superficial liposuction, with 22 (9%) performing liposuction only on this adipose layer. Eighty-five (35%) participants reported controlling the apparatus's pressure during the procedure. Conclusion: Liposuction procedures performed in Brazil have significant technical variations. This finding encourages us to reflect on the need to standardize liposuction practice and the manner in which it is taught so as to increase control over the procedure and its safety.

Introdução: A atividade leiteira é um sistema de trabalho que envolve diversas tarefas, que vão desde o manejo com o gado, alimentação, ordenha, até o consumo do produto final. A dinamometria isométrica de extensão lombar tem sido descrita como um bom instrumento de avaliação e acompanhamento laboral, sobretudo de populações que exercem muita força para desempenhar suas atividades. Objetivo: Avaliar a força isométrica de extensão lombar de produtores rurais na atividade leiteira. Métodos: A amostra contou com 47 trabalhadores rurais que exercem atividade leiteira cadastrados na Empresa de Assistência Técnica e Extensão Rural/Associação Sulina de Crédito e Assistência Rural do Rio Grande do Sul, que foram avaliados por meio de questionário sociodemográfico e testaram a força muscular de extensão lombar por meio de um sistema de dinamometria isométrica. Resultados: Participaram do estudo 20 homens com idade média de 50,45±9,44 anos e 27 mulheres com idade média de 46,15±7,56 anos. Quanto à força isométrica de extensão lombar, o grupo masculino alcançou 1169±289,96 N e o grupo feminino 571,34±190,36 N. Conclusão: Podemos concluir que o perfil da musculatura extensora lombar de trabalhadores rurais na atividade leiteira alcança valores superiores em indivíduos do gênero masculino, o que pode ser atribuído ao fato de homens apresentarem maiores níveis de massa corporal e consequentemente gerarem maior força


Background: Milk production is a work system that comprises several tasks from cattle handling, feeding and milking to product consumption. Isometric lumbar extension dynamometry is considered a satisfactory instrument for occupational evaluation and monitoring, especially for populations of workers whose activities demand high levels of strength. Objective: To investigate isometric lumbar extension strength among rural milk production workers. Methods: The sample comprised 47 rural milk production workers affiliated with the Technical Assistance and Rural Outreach Company/Southern Credit and Rural Assistance Company (Empresa de Assistência Técnica e Extensão Rural/Associação Sulina de Crédito e Assistência Rural ­ EMATER/ASCAR), Rio Grande do Sul, Brazil. The participants were analyzed based on a sociodemographic questionnaire and lumbar extensor muscle strength testing by means of an isometric dynamometry system. Results: Participants were 20 men with average age 50.45±9.44 years old and 27 women with average age 46.15±7.56 years old. The isometric lumbar extension strength was 1,169±289.96 N for the men and 571.34±190.36 N for the women. Conclusion: The lumbar extensor muscle profile of rural milk production workers was characterized by higher values for the men, which might be attributed to the fact that men exhibit higher body mass levels, and consequently produce greater force

Introdução: No presente trabalho, relatamos um caso de reconstrução do terço proximal do úmero direito e reanimação do cotovelo de um paciente masculino, de 20 anos, vítima de acidente automobilístico, com necrose óssea de 5 cm no terço proximal do úmero e avulsão do bíceps braquial. Métodos: Utilizamos o retalho ósseo da escápula, associado ao retalho miocutâneo do grande dorsal, tendo como pedículo os vasos subescapulares. Resultados: O paciente teve excelente evolução no pós-operatório, apresentando-se, no pós-operatório de 2 meses, com consolidação óssea e iniciando a flexão do cotovelo. Conclusões: Perante a utilização do retalho descrito, concluímos que esta modalidade de retalho se insere no arsenal dos retalhos ósseos de maior segurança nas reconstruções ósseas em geral.


Introduction: We report a case of reconstruction of the proximal third of the right humerus and rehabilitation of the elbow in a 20-year-old male patient who was injured in an automobile accident and developed bone necrosis of 5 cm at the proximal third of the humerus and avulsion of the brachial biceps. Methods: A scapular bone flap was used, together with a latissimus dorsi myocutaneous flap, using subscapular vessels for the pedicle. Results: The patient had excellent postoperative course, presenting in the 2-month postoperative period with bone consolidation and initiation of elbow flexion. Conclusions: This flap modality is a safe and useful option for bone reconstruction.

ABSTRACT

Objective:To observe the clinical effect of electroacupuncture (EA) at four sacral points on overactive bladder syndrome.Methods:A total of 120 female patients with overactive bladder syndrome were allocated to a treatment group of 80 cases and a control group of 40 cases on a voluntary basis.The patients in the treatment group received EA at four sacral points,and the treatment was given three times a week for 6 consecutive weeks,while the patients in the control group received oral administration of M-receptor antagonist tolterodine tartrate,which was given 4 mg each time,once a day for 6 consecutive weeks.Then the symptom scores were compared between the two groups before and after treatment.Results:At the end of treatment,the symptom scores showed statistical significant differences in comparing with those before treatment in both groups (both P<0.01);the symptom score in the treatment group was lower than that in the control group,showing a statistically significant difference (P<0.05).Conclusion:EA at four sacral points is an effective method for overactive bladder syndrome.

ABSTRACT

ABSTRACT Low back pain or lumbago is a painful condition that affects an increasing number of people around the world and has numerous causal factors. Among these factors, the most distinctive are muscle imbalances, weakness of paravertebral muscles and shortening of ischiotibial muscles. The role of these muscle groups is still controversial and not fully explained in the literature. Thus, a survey was conducted in the SciELO, LILACS, MEDLINE, and PubMed databases in the last decade (2003-2013), using as keywords: hamstrings, low back pain, lumbago, and paravertebral. The works found confirm that, according to the methodology, weakness in paravertebral muscles is of great importance to the development of low back pain as well as the shortening of the ischiotibial muscles, which are directly related to this factor.


RESUMO A dor lombar ou lombalgia é um quadro álgico que acomete um número cada vez maior de pessoas em todo o mundo e tem inúmeros fatores causais. Entre esses fatores, os que mais se destacam são desequilíbrios musculares, fraqueza dos paravertebrais e encurtamentos de isquiotibiais. O papel desses grupos musculares ainda é controverso e não está totalmente esclarecido na literatura. Desta maneira, foi realizada uma pesquisa nas bases de dados SciELO, LILACS, MEDLINE e PUBMED na última década (2003 a 2013), utilizando-se como descritores: isquiotibiais, lombalgia, dor lombar e paravertebrais. Os trabalhos encontrados confirmam que, de acordo com a metodologia empregada, a fraqueza dos músculos paravertebrais é de grande relevância para o desenvolvimento das dores lombares, assim como os encurtamentos dos isquiotibiais, que estão diretamente relacionados a esse fator.


RESUMEN El dolor lumbar o lumbago es una condición dolorosa que afecta a un número creciente de personas en todo el mundo y tiene numerosos factores causales. Entre estos factores, los que se destacan son los desequilibrios musculares, debilidad de los músculos paravertebrales y el acortamiento de los músculos isquiotibiales. El papel de estos grupos musculares aún es controvertido y no se explican completamente en la bibliografía. Por lo tanto, se realizó una encuesta en las bases de datos SciELO, LILACS, MEDLINE y PubMed en la última década (2003-2013), utilizando como descriptores: isquiotibiales, dolor de la región lumbar, lumbago y paravertebrales Los trabajos encontrados confirman que, de acuerdo con la metodología empleada, la debilidad de los músculos paravertebrales es de gran importancia para el desarrollo de dolor lumbar, así como el acortamiento de los isquiotibiales, los cuales están directamente relacionados con este factor.

ABSTRACT

SUMMARY Scleredema of Buschke (SB) is a rare disorder of connective tissue characterized by diffuse non-pitting induration of the skin, mainly on the cervical, deltoid and dorsal regions. It is a cutaneous mucinosis of unknown etiology and is associated with bacterial or viral infections, hematological disorders and diabetes mellitus. Histopathological examination shows thickened dermis with wide collagen bundles separated by gaps that correspond to mucopolysaccharide deposits, visualized using special staining. Several treatments are reported in the literature without well-established results. We report a case of SB in a patient with type 2 diabetes mellitus.


RESUMO Escleredema de Buschke (EB) é doença rara do tecido conjuntivo caracterizada por endurecimento difuso e não depressível da pele, principalmente nas regiões cervical, deltoideanas e dorso. Enquadrado no grupo das mucinoses cutâneas, tem etiologia desconhecida e associação com: infecções bacterianas ou virais, alterações hematológicas e diabetes mellitus. O exame histopatológico evidencia derme espessada com fibras colágenas calibrosas separadas por fendas que correspondem a depósito de mucopolissacárides, observados por colorações especiais. Diversos tratamentos são relatados na literatura sem resultados bem definidos. Descrevemos caso de EB em paciente com diabetes mellitus tipo 2.
